On this page you will find some of the Chapter Forms that you might need. (The forms are listed at the bottom of this
page, scroll down to find them.)
You can open, fill in, and print, the Chapter Events Release Forms. By doing this ahead of time, you will save time
and hassle in getting started on a Chapter ride, or event. Remember, that any non-Chapter member must
fill out one of these forms before they can be present at, and participate in, any Chapter event. Also take note that there
are separate Release Forms for "Adults" and "Minors".
If you are a non-Chapter Member and you wish to join us on one of our "Open"
Events, PLEASE fill out one of the Release Forms for yourself, any other adult coming with you, (i.e. your spouse), AND
any minors you might be bringing with you. Fill the forms out, print them, sign them and please bring them with you.
These Release Forms will also be available at the events and you can fill them
out there, if you forget to do it here.
NOTE! To fill in these forms, just open it, click your mouse in any shaded area of the form, then
fill in the information. When you're done, print the form out, sign it, and bring it with you. You can just print the blank
forms and fill them in by hand, if you so desire. If you see a form here without "shaded areas" on it, then you must print
that form as a "blank" and fill it in by hand.
